Accessible Document Request Form
We will consider reasonable requests to provide
information in accessible formats based on need
including, Large Print, Braille, Moon, Audio CD,
Audio tape, or computer CD.
If you have a need for specific information in one
of these formats, please complete this online request form
giving as much detail as you can about your needs. We will
endeavour to meet all reasonable requests and will acknowledge your
request in an appropriate manner according to your
preference.
We will keep you informed of the timescale of the fulfilment of
your request. This timescale will vary according to the nature of
the information that you have requested. If the document or
information is required for a specific date or meeting please give
as much notice as possible with the date of your deadline.
We reserve the right to decline to meet requests where it would not
reasonable or practicable to fulfil them but we will discuss this
with you fully with a view to reaching an agreement on an
alternative method of meeting your needs.
In order to be able to fulfil your request we must be able
to contact you as we may need further information before we can
decide how best to meet your needs.
