During a Councillor's term of office there is the possibility that his/her disclosed pecuniary interests (DPIs) may change.  When this occurs the Councillor must, within 28 days of becoming aware of the new DPI, notify the Monitoring Officer in writing.

A blank copy of a DPI form for a Parish Councillor can also be downloaded.

Completed copies should be completed electronically as editable PDF's, electronically signed as print and returned via email to:
